As part of the confiscation process in respect of Mrs Adedayo,
Post Office Ltd Investigation Team intend to conduct a
confiscation investigation. Accordingly I now write via you to
request from Mrs Adedayo authority for disclosure of Information
from financial institutions that she has dealings with.
In view of this please find enclosed 2 copies each of 2 partly
completed “Authority for Disclosure of Information” forms along
with 10 blank forms for such authorisation of any other accounts
to which Mrs Adedayo is a signatory. I am particularly,interested

Could you please arrange for Mrs Adedayo to see these forms
and if she agrees to give authority, ask her to complete as
appropriate and sign both copies of each disclosure form? I
would also ask that if authority is to be granted that I receive a
response within 14 days.
