1) I have asked that Mr/Mrs/Ms* + Solicitor, to attend this interview to give me
legal advice.

indicated they do not wish to be contacted*/ having been contacted, has declined to attend to attend the
interview*.

I have been offered the presence of another solicitor and informed that if I attend a police station voluntarily
(not under arrest) I would be entitled publicly funded legal advice. I have declined both of these options,
