Annex 3 - Summary Chronology of Key Events
The Inquiry requested a high-level chronology of the various steps POL has taken in relation to providing disclosure to the Inquiry. The body of the
Second Interim Disclosure Statement provides more granular detail. The below table is provided to assist the Inquiry in understanding the dates on which
key events took place.

1. January 2020 to March 2020 Peters & Peters commence a Post-Conviction Disclosure Exercise ("PCDE Review
2. September 2020 Following receipt of Data Subject Access Requests ("DSARs"), POL identified certain issues
relating to the way in which its historical documents were being stored at Winchester Oasis.
This was initially referred to as the '31,000 boxes’ exercise.
3. 19 and 23 October 2020 4,766 items identified at Winchester Oasis site as being 'unindexed' during the PCDE Review.
4. November 2020 HMC action plan put into place to address the 4,766 unindexed items.
5. November 2020 to January 2021 HSF carry out sampling exercise of the 4,766 unindexed items.
6. In and around 26 January 2021 The HSF sampling exercise completes.
7. 10 February 2021 POL stakeholders updated as to the outcome of the sampling exercise.
8. May 2021 HMC made the decision to complete a pilot exercise to inform stakeholders as to the content
of the 4,766 unindexed boxes. This involved 5% (237) of the 4,766 unindexed items being
indexed ("Pilot Exercise").
9. 1 June 2021 Statutory Inquiry commences.
10. November 2021 Pilot Exercise concluded.
11. 13 December 2021 First Rule 9 Request received that specifically engages phase 2 of the Inquiry, and, as such,
concerned potentially responsive material stored in hard copy document repositories.
12. 27 January 2022 HMC decided that a project should be initiated to index the remaining boxes in the 4,766
unindexed items.

23. Week commencing 23 May 2022 Finsbury Dials site visit carried out by POL to review both Finsbury Dials materials and
materials couriered from Swansea.
24. 23 May 2022 Winchester Oasis site visit to review materials couriered from Mutley.

40. 9 September 2022 Completion of phase 5 of the Unindexed Items Project, being the quality assurance exercise
of the Oasis Indices.
41. 13 and 16 September 2022 Items identified by HSF from the phase 1 Unindexed Items Project and responsive to Rule 9
Requests pertaining to phase 2 of the Inquiry were produced to the Inquiry.
42. 15 September 2022 Phase 4b of the Unindexed Items Project (the indexing of material from Chesterfield) largely
complete, save for a small handful of items subject to the Escalation Process.
43. 29 September 2022 HSF completes initial review of the indices from phases 3 and 4 of the Unindexed Items
Project that may be responsive to phases 2, 3 and 4 of the Inquiry.
44. October 2022 HSF commences electronic review of potentially relevant material digitised from phase 1 of
the Unindexed Items Project that may be responsive to phase 3 and 4 of the Inquiry.
