On Friday 25 November 2005 Mrs Adedayo was served with
summonses prepared.
A copy of the summons was given to Mrs Adedayo for her
retention along with form CS044. Mrs Adedayo was also given a
Means form and informed to complete it and return it to the court.
A notebook entry detailing the receipt of the Means form is
associated at Appendix C. The signed summonses have been
returned to North Kent Magistrates Court with copies associated at
Appendix C.
Mrs Adedayo is due to appear at North Kent Magistrates Court on
Thursday 22 December 2005 at 14.00pm.

now associated at Appendix C.
A Debarment notice (CS120) has been completed and was sent to
SSU on 27 October 2005.
