Re: Request for Authority for Disclosure of Information
Dear Miss Skinner
I currently have conduct of the confiscation investigation in your case. On 1°
June 2006 you signed Authority for Disclosure of Information for 2 Halifax
plc accounts. These were submitted to your branch and we received a
sponse from Halifax Customer Services re the account numbered
L _..I They did not respond to the other authority because the quoted
account number was incorrect.
Accordingly I write to you again to get a new authority to obtain disclosure
from the Halifax plc. I have associated with this letter 2 further copies of a
partly completed “Authority for Disclosure of Information” forms. If you agree
to give authority I would ask that you complete the forms and return them to
me.
I would also ask that you give me details of any other accounts with other
financial institutions to which you are a signatory and complete two
authorities for disclosure forms in respect each financial institution. The
exception to this is the following four financial institutions, as I do not require
further information from these;

I have enclosed some blank forms for your attention should you have other
accounts which I am currently not aware, if so could please sign two 2
copies of the authority for each and then return them to me in the enclosed
envelope.
In view of the pressing nature of this matter would you please respond to me
within 7 days of receipt? If you have any queries please do not hesitate to
