Re: Authority to Disclose
Information
As part of the investigation into in to the loss of £59,175.39
discovered at North Bransholme Post Office in May 2006 I am
investigation your suspected involvement Accordingly I now write to
you to request that you grant authority for disclosure of Information
from financial institutions that I believe you have dealings with.
In view of this please find enclosed 2 copies each of 2 partly
completed “Authority for Disclosure of Information” forms and several
blank forms.
If you agree to give authority please complete as appropriate
(including relevant address if known) and sign both copies of each
disclosure form? The blank forms are for authorisation of any other
accounts to you are a signatory and I request that you complete these
if relevant.
If you are a joint account holder could the other signatories sign the
relevant form to give their authority as well?
If you do consent could you please ensure that 2 copies of the
completed forms are returned to me within 14 days. If you have any
queries please do not hesitate to contact me on the number below. If I
do not hear anything within 14 days I will assume that you do not
consent to give such authority.
